Certification Criteria for LBP Diagnosis

In an effort to precede towards evidence based practice patterns for the treatment of low back pain, we will be requesting the following clinical information for members with LBP and related diagnoses.

On recertifications, the following may be asked for:

  1. Pain average and worst on 0-10 scale
  2. Centralization if applicable
  3. Use of modalities, yes or no
  4. Began active program?
  5. Oswestry updated score

Please try to include these criteria with all appropriate certification requests.
