Tobacco Cessation

The Geisinger Health Plan Care Coordination Tobacco Cessation Program is based on the 2000 Treating Tobacco Use and Dependence Clinical Practice Guideline developed by U.S. Department of Health and Human Services.

The following is a description of the Tobacco Cessation Program and its elements.

Program Goals

The primary goal of the Tobacco Cessation Program is to decrease the use of tobacco products among the Health Plan population, to include the use of smokeless tobacco, cigars, and pipe) through the following:

Interventions

Care Coordination nurses work with health care providers and rely on the following interventions when working with Health Plan members enrolled in the Tobacco Cessation program:

Program referral

To refer a Health Plan member to the Tobacco Cessation Program, please call (800) 883-6355 or (570) 271-8763 8 a.m. to 4:30 p.m.
