Care Coordination’s Hypertension Health Management Program is based on the Seventh Report of the Joint National Committee on Hypertension (JNC7).1

The following is a description of the Hypertension Health Management Program and its elements.

Program goals

The goal of the Hypertension Health Management Program is to minimize the morbidity and mortality of members with hypertension while achieving the highest possible quality of life through the following:

Interventions

Care Coordination nurses work with health care providers and rely on the following interventions when working with Helath Plan members enrolled in the HTN program:

Program Referral

To refer a Health Plan member to the Hypertension Health Management Program, please call (800) 883-6355 or (570) 271-8763 between the hours of 8 a.m. to 4:30 p.m.
