Geisinger Health Plan® - HeartWise Coronary Artery Disease Program
The primary goal of the HeartWise Program is to minimize the morbidity and mortality of members with coronary artery disease (CAD) while achieving the highest possible quality of life through the following:
Incorporation of the CAD Guideline into a comprehensive Health Management program.
Increase the rate of beta-blocker therapy among members with CAD.
Increase the use of aspirin therapy among members with CAD.
Increase the evaluation of lipid levels among members with CAD.
Increase the use of lipid-lowering therapy for members with CAD.
Improve ASCVD risk factor control among members with CAD.
Interventions
Care Coordination nurses work with health care providers and rely on the following interventions when working with Health Plan members enrolled in the HeartWise program.
Collaboration with Primary Care Provider to promote best practiceaspirin therapy, lipid evaluation, blood pressure and lipid management, ACE therapy, risk factor reduction, and referrals to specialists as indicated.
Health promotion/disease prevention servicesinfluenza and pneumococcal vaccines, mammograms, safety belts, etc.
Data collection to support outcomes managementLDL and B/P values, ASA use, and ACE/ARB use.

To refer a Health Plan member to the HeartWise Program, providers may call (800) 833-6355 or (570) 271-8763, Monday through Friday, 8 a.m. to 4:30 p.m.
