Health management is the application and coordination of resources for members with a chronic condition. These resources are provided across the continuum of care. Geisinger Health Plan offers a variety of health management programs designed to improve the health status of your members. Health management nurses will work with primary care physicians to coordinate care and services of your members enrolled in the health management programs.
