Get your reimbursement faster and easier with EFT. Claim payments will be directly deposited to the identified provider’s bank account. No more checks to handle.

Once this form is received we will validate your bank account and routing information by sending a pre-note to your bank. Once your bank account information has been verified we will contact you to explain when to expect your first EFT transaction.
Fast Facts
Paper explanation of payment (EOP) will still be generated and distributed by mail, however, no check will accompany the EOP.
EFT payments can start in as little as two weeks. You will be notified prior to this occurring.
EFT payments for all line of businesses expect Third Party Administrator (TPA) are processed Mondays(except Bank Holidays).
TPA transfers will be made when funded. This is the same as without EFT.
