Case Management

The Care Coordination Department has made several program changes in order to enhance the delivery of disease and case management services.

Care Coordination services will now be called Case Management services and Care Coordination nurses will now be referred to as Case Managers. The Case Managers will continue to work directly with you and your Geisinger Health Plan patients to provide post hospital follow-up, identify and assess resource issues, and deliver the appropriate health management services. Focusing on these three areas will improve the program with a more integrated and targeted Case Management approach.

Case Management is a Geisinger Health Plan department that promotes health care strategies proven to improve population-based, humanistic and financial outcomes.

This is accomplished through:

Care Coordination nurses who work with primary care physicians to provide member education and case management services to Health Plan members with chronic and complex health conditions. Health management programs designed to improve the health status of members who have chronic health conditions.

Coordination of health management care and services with the primary care physician. Clinical guidelines which offer consistent and appropriate guidance for diagnosis and treatment of specific conditions or diseases.
