Health Management Programs

Health management is the application and coordination of resources for members with a chronic condition. These resources are provided across the continuum of care.

Geisinger Health Plan's Care Coordination Department offers a variety of health management programs designed to improve the health status of your members. Care Coordination nurses will work with primary care physicians to coordinate care and services of your members enrolled in the health management programs.

Health management programs available through Care Coordination are listed below. You can view information about each program online by clicking on your selection below.

  • Asthma Care Program
  • Chronic Kidney Disease Program
  • Chronic Obstructive Pulmonary Disease (COPD) Program
  • Congestive Heart Failure Program
  • Diabetes Care Program
  • HeartWise Coronary Artery Disease Program
  • Hypertension Program
  • Osteoporosis Program
  • Tobacco Cessation Program
