Care Coordination
Update (March 2006)
The
Care Coordination Department has made several program changes in order to enhance the delivery of disease and case management services.
Care Coordination services will now be called Case Management services and Care Coordination nurses will now be referred to as Case Managers. The Case Managers will continue to work directly with you and your Geisinger Health Plan patients to provide post hospital follow-up, identify and assess resource issues, and deliver the appropriate health management services. Focusing on these three areas will improve the program with a more integrated and targeted Case Management approach.

Asthma care, chronic obstructive pulmonary disease (COPD), osteoporosis and coronary artery disease were accredited in 2005. Congestive heart failure, hypertension and diabetes care were accredited in 2003.
