Case Management
Case Management is a vital component of Care Coordination with services designed to work with both you, the provider, and with members who have chronic health problems and/or psychosocial issues that require individualized attention. While assisting you with your members, Case Management nurses will keep you informed and involved in guiding your members' care.
Interventions by Case Management may include patient/family education, referral to community resources, coordination of on-going care, home assessment, continual follow-up and evaluation/placement into long-term care or hospice. Benefits of Case Management include a multi-disciplinary approach and efficiency for both you and your members.
