Breast Cancer Screening
This HEDIS measure is the percentage of women 50-69 years of age who had a mammogram within the last two years.
Beta-Blocker Treatment After a Heart Attack
This HEDIS measure is the percentage of members 35 and older who were discharged from the hospital after surviving a heart attack and who received a prescription for beta-blockers.
Childhood Immunization Status – Combo #2
This HEDIS measure is the percentage of children who by age two have received: 1) Four diphtheria/pertussis/tetanus (DTaP/DT) immunizations; 2) Three polio virus (IPV) immunizations; 3) One mumps/measles/rubella (MMR) immunization; 4) Three hemophilus influenza type B (HiB) immunizations; 5) Three hepatitis B (Hep B) immunizations; and 6) At least one chicken pox (VZV) vaccination
Child Access to Primary Care (12-24)
This HEDIS measure is the percentage of children 12–24 months of age who saw a health care provider for primary care in the preceding year.
Adolescent Immunization Status – Combo #2
This HEDIS measure is the percentage of adolescents who received: 1) A second dose of mumps/measles/rubella (MMR) immunization; 2) Three hepatitis B (Hep B) immunizations; and 3) One chicken pox (VZV) vaccination by their thirteenth birthday
