Health Plan Names Recipient of annual nursing award

DANVILLE, PA–Geisinger Health Plan recognized Sonia Hoffman, RN, BSN, with Geisinger Health Plan's annual nursing award during a celebration for health plan nurses. The nursing award and celebration marked the end of National Nurses Week at the Health Plan.

Each year, the award is presented to a Health Plan nurse who has been recognized for providing outstanding quality care and customer satisfaction. This award was originated and funded in 2002 by a Geisinger Health Plan member in recognition of Lisa Mattive, LPN, benefit nurse coordinator for Geisinger Health Plan.

Hoffman was nominated by her peers for her exceptional work ethic, and the dedication and compassion to members that she works with in her position as case manager. “Sonia is a great asset to Geisinger Health Plan,” says Janet Tomcavage, RN, vice president of health services. “She was the first case manager hired for our new Personal Health Navigator initiative and she has been instrumental in the success of this program.”

Hoffman graduated from the Geisinger School of Nursing in 1994 and has been an employee of Geisinger Health System since 1993. She spent the last seven years working for the Health Plan, first as a health manager in the care coordination department. In her current position as a Personal Health Navigator case manager, Hoffman helps coordinates care by directing patients through the complex health care system including primary and specialty care, nursing home care, hospital and home health care.

“I am honored to be chosen for this award. Because I know the caliber of nurses who work at the Health Plan, winning this award is even more special to me,” says Hoffman. “But the greatest reward for my job is the thanks that I receive from the members and their families. That makes it all worth it.”

Hoffman resides in Milton with Scott, her husband of 10 years, and three children, Cassidy, 8, Sydney, 4, and Kelsey, 2.
