Our team of medical professionals helps members manage chronic conditions and complex medical needs.

Health Management

Current members with chronic conditions are encouraged to enroll in one of our health management programs. This includes important self-management education designed to help you live a healthier life, with guidance from our case managers, who are registered nurses. Our health management programs include asthma, chronic obstructive pulmonary disease (COPD), diabetes, heart failure, coronary artery disease, hypertension (high blood pressure), osteoporosis, and tobacco cessation.

Our team of nurse managers works with other health care providers and to help current members manage complex medical situations.

Asthma Center

Asthma sufferers can learn to manage their symptoms with these resources for avoiding flare-ups and dealing with allergies. There are also ways to monitor your child’s asthma medications and a helpful glossary for learning important asthma-related terminology.

Diabetes Center

A diabetes diagnosis can come with many questions. Learn the basics about insulin and how it affects your body, what happens when you have diabetes, and what you can do to help your child control and manage diabetes. There are also printable food diary and blood glucose charts.

Cancer Center

This helpful guide to dealing with cancer includes treatment and prevention options, facts about common medical testing and information on different types of cancer. Connect with others through personal stories and help your family learn coping strategies that will guide them through difficult days.
