Diabetes LDL-C Testing
This HEDIS measure uses a single sample of members with diabetes to estimate the percentage of members with diabetes who received lipid level (LDL-C) testing.
Diabetes HbA1c Poorly Controlled >9.0% (a lower rate indicates better performance)
This HEDIS measure uses a single sample of members with diabetes to estimate the percentage of members with diabetes who received Hemoglobin A1c (HbA1c) testing and whose results were not controlled >9.0%.
Breast Cancer Screening
This HEDIS measure is the percentage of women 40-69 years of age who had a mammogram within the last two years.
Persistance of Beta-Blocker Treatment After a Heart Attack
This HEDIS measure is the percentage of members 18 and older who were discharged from the hospital after surviving a heart attack and who received persistent beta-blocker treatment for six months after discharge.
Childhood Immunization Status – Combo #2
This HEDIS measure is the percentage of children who by age two have received: 1) Four diphtheria/pertussis/tetanus (DTaP/DT) immunizations; 2) Three polio virus (IPV) immunizations; 3) One mumps/measles/rubella (MMR) immunization; 4) Two hemophilus influenza type B (HiB) immunizations; 5) Three hepatitis B (Hep B) immunizations; and 6) At least one chicken pox (VZV) vaccination
Child Access to Primary Care (12-24)
This HEDIS measure is the percentage of children 12-24 months of age who saw a health care provider for primary care in the preceding year.
Adolescent Well Care
This HEDIS measure is the percentage of members 12-21 who had at least one comprehensive well-care visit with a PCP or an OB/GYN in the last year.
